also needed on the applicant's part are a passport in good condition and valid not less than 6 months form the expected date of visa issue, certified copies of page containing annotations, one signed visa application form completed electronically on this page (on link 'solicitar visto'), three recent passport-type photographs, a legalized certification of non-criminal record from city of residence in a sealed envelope, and documents attesting to the legality and registration of all document translators. 

furthermore, a document proving the professional experience or qualifications compatible with the activities to be exercised, duly legalized by the notary public and the Consulate General of Brazil - This document must be written in English or Portuguese. 

finally, a document showing proof of permanent residence must be provided, along with copy of a round-trip ticket to brazil, and appropriate consular fees.
