To protect quality of life and functional autonomy, the health guidelines of Elder (2008) propose that the elderly should have access to the public health system through the family health strategy (ESF) and basic health units (UBS), and teams should be prepared to receive them and tend to them with priority service and an adequate structural procedure. This work aims to analyze how the health of elderly hypertensive patients registered by family health teams of VilaVintém of CMS of Padre Miguel is monitored. In this research the methodological design approach is a qualitative case study, with a descriptive purpose. The procedures used were: semi-structured interviews, applied to health-care professionals of the five teams of the SF of Vila Vintém Municipal Health Center (CMS) of Padre Miguel in the neighborhood of Padre Miguel in Rio de Janeiro; selection and analysis of secondary data provided by SIAB (Basic care information system), through the electronic health record VITACARE filled out by the teams to obtain the profiles of elderly hypertensive patients registered.
